For your convenience we have included a link below to our new patient paperwork allowing you to complete it in the comforts  of your own home or office.

  • Fax us your printed and completed forms or bring them with you to your appointment.

New Patient Information and Health History Forms - Required

Our new patient forms provide us with the needed information about your current state of your health along with what questions, concerns or goals regarding wellness we can help you with?  Please print all pages of New Patient Forms and complete prior to your first visit appointment. If the reason for your visit is due to a car accident please also include those forms. After completing you can fax them to 970-352-2006 or bring them with you to your appointment.

I am pleased to tell the fantastic recovery I experienced while under Dr. Scott's care. I had twisted stuff in my leg as a result of tripping over a threshold. I had major pain and major weakness in that leg. Dr. Scott found the right combination of adjustments to get me on the right road to recovery. It took a couple of months but it kept improving. Now 6 months out from my last appt. (Oct 2011). I have not had set backs. Strength has maintained. I can't express enough how well I've been. Thank God and Dr. Scott!!!
